Job Description:

The Home Health Registered Nurse performs a comprehensive patient assessment and develops an individualized patient plan of care with collaboration of an interdisciplinary team and physician approval. - -Under the direction of and in accordance with the physician's orders administers skilled nursing care to patients in their place of residence. - -Functions as a case manager and collaborates with the interdisciplinary team to meet patient outcomes and goals. - -As a leader of the healthcare team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. - -Demonstrates performance consistent with the PIH Health vision of Patients First.

Required SkillsHome Health -

• Establish positive and professional relationships with patients, families, staff, and physicians.
• Demonstrate quality nursing clinical practice while meeting time-sensitive deadlines.
• Knowledge and ability to apply the nursing process while developing an individualized patient plan of care.
• Able to demonstrate clinical nursing expertise in the establishment of peripheral intravenous lines and administration of intravenous therapies.
• Computer skills and ability to navigate an electronic health record. -
  Required ExperienceHome Health Only -
  • Current California license as a Registered Nurse in good standing.
  • A minimum of one year experience as a professional nurse, preferably in an acute care facility.
  • BSN referred.
  • Current CPR card
  • Current California driver's license and valid auto insurance.
  • Home Health experience preferred. -
