Job Description:
A company is looking for a Care Manager (RN) Diabetes.
Key Responsibilities
Develops and facilitates complex care management activities and
personalized care plans for members
Coordinates care between members, families, and providers to ensure
appropriate person-centered services
Provides education to members and families on disease processes and
healthcare options
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Experience in diabetes education strongly preferred
