Key Responsibilities
Supervise the care coordination team for long-term care members
Monitor compliance with federal and state regulations and manage
escalated care cases
Provide coaching, training, and performance evaluations for the
care management team
Required Qualifications
Bachelor's degree
4+ years of related experience
Experience in care management or long-term care services
Ability to lead and facilitate change
Experience in team performance evaluation and coaching
